      WHO ARE THE ELIGIBLE CANDIDATES FOR LUMBAR DISC HERNIA (DISCECTOMY) OPERATION?

      Aging, sedentary lifestyle, obesity, occupation and genetic factors play a role in the formation of a lumbar disc hernia. Symptoms include constant numbness and pain in the legs and feet, difficulty in moving, loss of balance and foot wasting, constant waist pain and muscle weakness.

      HOW IS LUMBAR DISC HERNIA (DISCECTOMY) PERFORMED?

      The purpose of this surgery is to remove the herniated lumbar spine discs and relieve the pressure on the nerve. There are 3 methods to perform this surgery: Open Discectomy, Endoscopic (Closed) Discectomy and Microdiscectomy. This surgery, performed under general or spinal (epidural) anesthesia, takes between 40 minutes and 1 hour.

      • PREPARATION

      The first and crucial step is to have physical examination. During the physical examination, doctor performs manual examination and moving examination. According the findings in the physical examination, surgery is planned. After the planning of surgery, the patient have to undergo pre-operative tests and consultations such as learning of patient’s medical record, blood tests, chest X-ray, waist X-ray, computed tomography (CT Scan), magnetic resonance imaging (MRI) and anesthesiologist consultation.

      According to the patient’s medical record, surgeon may ask for additional tests and examinations from different branches.

      The main aims of pre-operative examinations are evaluate the patient’s risk and maitain patient’s health condition.

      • ANESTHESIA

      Spinal (epidural) anesthesia which is numbing the lower body from waist or general anesthesia which is numbing whole body and put patient to sleep are preffered. The anesthesiologist will decided the which type of anesthesia is suitable for the patients.

      • LUMBAR DISC HERNIA (DISCECTOMY) OPERATION TECHNIQUE

      It can be performed in three different ways: open discectomy, endoscopic (closed) discectomy and microdiscectomy.

      In the open method, the patient is placed on the operating table in a prone position and this method is performed under general anesthesia. An incision of 2 to 4 cm is usually made in the waist area. A small area is opened from the bone tissue under the spine to access the underlying ligaments and tissues. Usually, these connective tissues are either removed or the herniated part that is pressing on the nerve is removed. The operation is terminated after the surgical procedure is completed.

      In the endoscopic method, general or spinal anesthesia can be applied. It usually starts with 3 or 4 small incisions made in the area where the herniated disc is located or 10 cm from the side of the hernia. Laparoscopic surgical instruments are inserted through the incisions and the inside of the body is imaged. Since bones and muscles are generally not touched in this procedure, these structures are preserved. The operation is terminated by removing the herniated tissue.

      In the microdiscectomy method general anesthesia is preferred.The surgery usually starts with a small incision of 2 cm. Special surgical instruments are inserted through this incision. The procedure is performed with imaging surgical instruments, operating microscopes, and microsurgical hand tools. Thanks to these devices, nerves and nerve membranes are not damaged. The herniated tissue is reached and removed. The operation is terminated after the surgical procedures are completed.

      RECOVERY FROM LUMBAR DISC HERNIA (DISCECTOMY) OPERATION IN TURKEY

      Patients stay at the hospital for 1 or 2 nights after the surgery. Patients are recommended a waist corset to support their waist after surgery. And at the same time, individuals who have had surgery are not recommended to go up and down stairs for 3 days.

      Patients should take plenty of time to rest after returning home. Individuals who have had surgery are not recommended to lie down on the couch and they are not asked to lie in the same position all the time. It is important to keep the waist bent while standing and sitting after the surgery. If individuals are overweight, they are advised to lose weight to prevent recurrence of hernia.

      After surgery, patients are recommended physical therapy. It is very important to start physical therapy 2 months after the surgery. Physical therapy aims to heal and strengthen the tissues and muscles in the lumbar area.

      Although the healing process varies from individual to individual, it is generally observed within 4 to 6 weeks.
