In compliance with your selected revitalization and surgical facilites, you will ensue with the further step of filling the Medical History Form. The medical history together with any additional medical requirements will allow the surgeon to make initial medical evaluations. It is crucial that you provide us all the information requested in order our doctors to deliver the most efficient treatment.

| Step 3 : Surgical Consent Forms |
After you have initially submitted below forms and guided through the last-steps of the data-accumulation phase, you will be directed to the Surgical Consent Forms section. Each of the surgical operation has a Surgical Consent Form which intends to highlight all the aspects of the selected surgery. It is important that you read the procedure consent carefully and have all your questions answered at your consultation prior to surgery.

| Step 4 : Booking Form |
The last step is the Booking Form Step, After you are comfortable with your choice and would like to confirm your provisional booking, we require that you forward the completed booking form. The pre & post operation instructions for your selected procedure/s and invoice for payment of the non-refundable deposit will be forwarded to you. On receipt of the funds we will confirm your bookings and forward a detailed itinerary for your surgery and medical appointments.
